At Queens Boulevard Extended Care Facility, we understand that the needs of our Residents and their families are varied and complex. This is why we provide support and counseling services for each resident from the time of admission and throughout their stay at the facility. A Social Worker is assigned to each resident, enabling us to fully understand the richness of their lives and the issues that determine their need for short or long-term placement. This personalized approach helps our team meet each resident’s individual needs.
If appropriate, the Social Worker will work with a resident to design a plan for a safe discharge back to the community. For those residents who remain with us for long-term care, the Social Worker is readily available to provide ongoing support. Our Social Work Department is always ready to help promote the best quality of life possible for each of our residents.
