Strategically located minutes from major highways, buses, subways and the Long Island Railroad, our facility is easily accessible from the five Boroughs, Long Island, and Westchester County.

Directions By Car:
From Brooklyn or Staten Island:
Take Belt Parkway Exit 17 to Crossbay Boulevard North. Continue on Crossbay Blvd., which turns into Woodhaven Blvd. Take Woodhaven Blvd. to Queens Blvd. (the Queens Center Mall will be in front of you). Make a left onto Queens Boulevard and stay in the outside lanes. Queens Boulevard Extended Care Facility will be several blocks down on your right.

Or
Take the Brooklyn Queens Expressway (BQE) East to Exit 39 toward NY-25/Queens Blvd. Turn left on 65th Place. Take the 2nd left onto Queens Blvd. Take the 1st right onto 63rd Street.
Turn left onto Queens Boulevard. The Facility will be on the right.

From Long Island:
Take the Long Island Expressway to Exit 19 toward Queens Boulevard. Turn right onto Queens Blvd. Continue approximately 1.7 miles. The Facility will be on the right.

From The Bronx:
Take the RFK (Triboro) Bridge to I-278W toward Brooklyn/Staten Island. Take Exit 39W (NY 25W/Queens Blvd.). Merge onto Queens Boulevard, the Facility will be on the right.

From Manhattan:
Take the Queensboro Bridge and follow signs for Queens Blvd. Continue on Queens Blvd. for approximately 3 miles. The Facility will be on your left.

By Subway:
Take the 7 Train (Local or Express) to 61st Street/Woodside. Walk three blocks south to Queens Boulevard.

By Long Island Railroad:
Take the LIRR to the Woodside Station. Walk three blocks south to Queens Boulevard.

By Bus:
Take the Q60 bus to Queens Boulevard. The bus stops outside the Facility on 63rd Street.
